Most Live Scans also have required government fees in addition to the rolling fee. Please be sure to bring a valid unexpired government photo ID along with your completed Live Scan Request form to your appointment.
Please be sure to bring a valid unexpired state driver license, state identification card, or passport for each signer. Have all paperwork to be notarized filled out ahead of time with the exception of your signature.
